U5-U8 Reschedule Request Form
Coaches may submit this form to request a reschedule for a game (certain guidelines apply).
Reschedule Procedures:

Send request to applicable age commissioner if request(s) meet the following criteria:

Reschedules are only granted for teams unable to make minimum player requirements for reasons due to unforeseen religious or school events.

Reschedule requests shall be accepted if sent before a week of the scheduled game. E.g. Game – 4/12, request sent before 4/5.

Exceptions to “1.i. and 1.ii.” are determined by the applicable age commissioner.
Age Commissioner will submit request to U5-U8 Scheduler for processing. U5-U8 Scheduler will process, if able, and send confirmation to applicable Age Commissioner and Coaches/Team Managers.
